Yes! Please download and fill-out both the New Client Enrollment Form and my Disclosure Statement Form, and bring them to your first appointment.
Part of early therapy is doing evaluation work, and below are a number of tools I use for such purposes. Feel free to download and complete the assessments below, but also know it is fine waiting until we meet to discuss your situation if you would prefer.
My Comprehensive Behavioral Health Self-Evaluation Form is designed to help you understand your life challenges more clearly, and to help me better know how to help you. It is comprehensive in that it evaluates many domains of your life, and because of that, it may take you up to an hour (sometimes longer) to complete. There is no need to do it all in one setting, so feel free to take breaks if you want. Remember, all your answers are covered by our confidentiality agreement.
In addition to the above form which evaluates your life challenges, I also like to assess your character strengths as well. To do this, I utilize a free version of the Values in Action (VIA) Survey offered by the VIA Institute on Character. Please note that the initial results are free, but the site does allow you to purchase more detailed reports for a cost of $20-$50 depending on how much detail you want. For our purposes, the free version is fine, but whatever version you go with, please print out the results and bring them to your session.
Finally, I believe our work together should produce measurable results. Therefore, I use a tool called the Outcome Questionnaire 45 (OQ-45) to assess your degree of symptoms and functioning at the start of treatment, and then re-administer it periodically to see how we are doing in our work together. It takes about five minutes to complete.
